Routine Home Care
We provide all services on a weekly basis in patient’s homes, residential care facilities for elderly, assisted living facilities, and skilled nursing homes.
General Inpatient Care
Unity First Hospice provides general inpatient care for patient in crisis and in need of pain control and symptom management that cannot be provided in home settings. We have contacted skilled nursing facilities and local hospitals where patients can receive appropriate short-term inpatient care and return home on routine level of care time, generally just a few days.
Continuous Level of Care
Our team provides continuous level of care indicated for patient in crisis 24 hours a day to achieve palliation or management of acute medical symptoms for brief periods of time, generally just a few days.
Limited to five consecutive days, respite care provides a brief ‘respite’ for the patient’s primary caregivers by admitting the home care patient to an institutional setting without meeting the ‘inpatient’ pain and symptom management criteria.
Eligibility requirements for hospice will be determined when a patient meets the following criteria – the patient has a chronic illness such as: Alzheimer’s, ALS, Cancer, Cardiac Disease, Debility, Dementia, HIV/AIDS, Liver Disease, Renal Disease, Stroke.
The patient has terminal diagnosis with a limited life expectancy of 6 months or less if the conditions take its natural course.
The The patients physician and hospice physician authorize hospice care.
